Observed cooked chicken wings in two rectangle bins without coverings sitting on the counter. Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. COS PIC put coverings on the chicken wings.

Observed facility operating without verifiable employee health policies for all employees. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. COS PIC was given 6 blank employee health policy papers for the employees to read and sign.

Observed chlorine sanitizer bucket reading 10 ppm. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions P, and shall be used as follows:

1. A chlorine solution shall have a minimum temperature based on the concentration and pH of the solution as listed in the following chart; P

Person in charge was given employee health policy. Corrective Action: PIC provided copy to review and distribute to employees.

Observed chlorine bleach sanitizer above 200ppm. Rule: Chemical sanitizers, including chemical sanitizing solutions generated on-site shall:

1. Meet requirements specified in 40 CFR 180.940 Corrective Action: PIC made new chlorine bleach solution at 50ppm.

Observed inspection score of 99 A posted. Rule: The most current inspection report shall be prominently displayed in public view at all times. Corrective Action: PIC given a copy of today's inspection to post.

Observed shrimp and fish stored above sauces in walk-in cooler. Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables. Person in charge re-arranged walk-in cooler to prevent cross contamination.

Person in charge was given employee health policy.

Observed facility lacking handwashing signage at handwashing sinks located in the main kitchen area and the warewashing area. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. Person in charge posted handwashing signage.

Observed female restroom without covered receptacle. (h) Toilet Room Receptacle, Covered. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. Corrective Action: PIC will place covered trash can in restroom.
