4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed several TCS food items in the walk-in freezer uncovered. Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. PIC covered TCS food items in walk-in freezer.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(8)(b) - hot water and chemical-methods(p)
Observed food employee actively washing dishes and sanitizing them in 3-compartment sink with no sanitizing solution at all in the 3-compartment sink. After being cleaned, equipment food-contact surfaces and utensils shall be sanitized in:

Hot water manual operations by immersion for at least 30 seconds in water at a temperature of 171°F (77°C) or above; P

Hot water mechanical operations by being cycled through equipment that is set up as specified under subsections (6)(e), (l), and (m) of this Rule and achieving a utensil surface temperature of 160ºF (71ºC) as measured by an irreversible registering temperature indicator; P or
-
Chemical manual or mechanical operations, including the application of sanitizing chemicals by immersion, manual swabbing, brushing, or pressure spraying methods, using a solution as specified under subsection (6)(n) of this Rule. Contact times shall be consistent with those on EPA-registered label use instructions by providing:
(i) Except as specified under paragraph 3(ii) of this subsection, an contact time of at least 10 seconds for a chlorine solution specified under subsection (6)(n)1 of this Rule; P
(ii) A contact time of at least seven seconds for a chlorine solution of 50 ppm that has pH of 10 or less and a temperature of at least 100ºF (38ºC) or a pH of 8 or less and a temperature of at least 75ºF (24ºC); P
(iii) A contact time of at least 30 seconds for other chemical sanitizing solutions; P or
(iv) A contact time used in relationship with a combination of temperature, concentration, and pH that, when evaluated for efficacy, yields sanitization. P
PIC attempted to refill the sanitizer mechanism but realized they did not have any more solution specifically for the 3-compartment sink. PIC was told to utilize the warewashing machine until they are able to properly use the 3-compartment sink. At the time of the inspection the warewashing machine was working properly.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed employees operating in the main kitchen without proper hair restraints. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. PIC instructed employees to wear proper hair restraints.
