4-2B: .05(7)(a)1Observed interior of the drink machine and drink nozzles with a black mold like substance. Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf) Corrective Actions: Drink machine and nozzles washed, rinsed, sanitized and allowed to air dry prior to returning into operation.

12B: .03(5)(h)Observed grill operator prepping grilled sandwiches with a green rubber bracelet on his left arm. Food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food (except for a plain ring such as a wedding band). (C) Corrective Actions: Grill operator removed bracelet during corrective action for beard guard mentioned prior and washed their hands prior to returning to food prep.

12B: .03(5)(j)Observed grill operator prepping grilled sandwiches with a beard/goatee longer than 1/2 an inch without a beard guard. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch (1/2"). (C) Corrective Actions: Employee put on a beard guard provided by inspector and washed their hands prior to continuing food prep.
