All cold and hot held items were in compliance, unless noted on the temperature chart below. All food in freezers frozen solid. Approved Additives: Vanilla Extract An unscored informal will be conducted on or before May 8th, 2026 to ensure all violations are corrected. Failure to correct this violation may result in permit suspension. Note: Even though the facility has a cleaning service that cleans up if a vomit or diarrhea event occurs, the kitchen must still have a clean up kit with an approved sanitizer that can be used by the kitchen staff in the even the clean up crew is unavailable. 6-1A: .04(6)(f)Observed multiple TCS items, such as sliced tomatoes, sliced cheese, deli meats, shredded lettuce, shredded cheese, being held in the prep cooler located across from the grill in the front part of the kitchen near the register Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P) Corrective Actions: Person in Charge (PIC) discarded the food items.

6-2: .04(6)(g)Observed several foods that the PIC stated had been prepped yesterday that did not bear a prep or discard date. These items included deli meats, diced chicken, and shredded lettuce. Refrigerated, ready-to-eat, time/temperature control for safety foods prepared and held in a food establishment for more than 24-hours shall be clearly marked to indicate the date of prep or the date by with the food shall be consumed, sold, or discarded when held at 41°F (5°C) or below for a maximum of 7 days. The day of preparation shall be counted as Day 1. (Pf) Corrective Actions: PIC labeled all food items with the correct date.

15A: .05(6)(a)Observed the prep cooler located at the front of the kitchen near the register is holding an ambient temperature of 47 degrees. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections .05(1) and .05(2) . (C) Corrective Actions: PIC will not use this cooler for TCS items. If the PIC used the cooler for TCS items, the items will be placed in an ice bath to keep cold and PIC will monitor the temperatures throughout the shift until the cooler has been repaired to hold 41 degrees or below.
