Plan of Correction: The facility has 30 days to make all corrections on the above listed rooms as they have been put out of service until access is granted by the health department. The facility must work closely with their pest control company to eliminate all pests and insects. All damaged and soiled mattresses and mattress covers must be discarded and replaced. A receipt must be provided to the health department when asked. All mold must be properly treated and cleaned; a receipt of service must be provided. All floors and walls must be fixed and cleaned to the sight and touch; replacing any damaged cove base. All damaged and missing AC units must be replaced or the area must be tightly sealed. All hot water in the guest room must be turned down to maintain a temperature below 121 degrees F; not to exceed 120 degrees F. If the corrections are not completed with in the next 30 days, by October 24, 2024, the facility's permit may be suspended and legal actions may be enforced to suspend all rooms. Rooms 103, 104,105, 108, 123, 207, and 225 are currently suspended and can not be used for services until granted by the health department. Violation of this order will result in a citation.

There were multiple rooms where insects were crawling under the floor tiles, light switches, in the stove, sink, mattresses and in the cracks of the walls. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Corrective Actions: All rooms will remain suspended until proper treatment is done to eliminate pests in all rooms.
