8: .15(5)Observed that the facility's annual gas-fire equipment inspection had expired in September. Facility did not have a current annual-gas equipment inspection. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency. Corrective Actions: NCOS

1: .07(2)Observed room 225 with hot water exceeding temperatures over 120 degrees F. Hot water in all guest rooms shall not exceed 120ºF. Corrective Actions: **The proper adjustments must be made to ensure all hot water temperatures does not exceed a 120 degrees F in all guest rooms. **

6: .13(1)Observed an infestation of insects in all of the rooms inspected today. There were multiple rooms where insects were crawling under the floor tiles, light switches, in the stove, sink, mattresses and in the cracks of the walls. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Corrective Actions: All rooms will remain suspended until proper treatment is done to eliminate pests in all rooms. The facility can not rent these rooms out until access is granted by the health department.

24: .13(3)There were multiple rooms where the AC unit has been removed. This has created a large opening into the rooms for pests and insects to easily enter the rooms. [.13(3)] Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. Corrective Actions: Violation corrected. The covers are not all the way attached but they are on in a way to protect insects and rodents from entering the rooms.

25: .14(1)Observed mold on the ceiling in the shower and the bottom wall damaged and cove base has fallen off in room 105. Observed mold on the ceiling in room 207. [.14(1)] Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored. Corrective Actions: **All rooms must be properly repaired and cleaned to the sight and touch by the indicated timeframe. All rooms has been suspended for service until access is granted by the Health Department.

28: .14(6)Observed rooms 123, 207, and 225 with soiled mattresses. Those rooms that had new mattresses did not have mattress covers for protection of the mattress to eliminate it from being soiled. [.14(6)] Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly. Corrective Actions: All soiled mattresses, mattress covers, must be replaced. New mattresses must be protected by mattress covers. All rooms will remain suspended until clearance is given by the health department.

33: .17(2)Observed a Coca-Cola and a propel water stored on two separate house keeping carts with clean items for the rooms directly over clean linens. Observed an employee lunch bag and personal tumbler cup stored on the linen rack directly over clean linen. [.17(2)] Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Corrective Actions: Education was provided to the staff discussing proper storage of personal items to prevent contamination. All items were relocated.

28: .14(6)Observed in room 331 that the mattress cover was stained, the comforter had burn holes and the sheets had stains. Observed in room 346 that the mattress cover had stains. Observed in 337 that the sheets and mattress cover had stains [.14(6)] Washable mattress pads or covers shall be used on all mattresses.
