1-2A - pic present, demonstrates knowledge, performs duties

Upon my arrival, I was told that there is not a designated person in charge. CA: There must be a designated person in charge during all operational hours.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(b) - hand drying provision (pf)
Observed that the rear hand wash sink is out of paper towels. CA: There must be paper towels located at each hand wash sink. COS: The rear hand wash sink was supplied with paper towels.

Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed a dirty whisk placed inside of the front hand wash sink. CA: Hand wash sinks are only used for hand washing. COS: Whisk was removed from the hand wash sink.
6-2 - proper date marking and disposition
Regulation: 511-6-1.04(6)(h) - ready-to-eat time/temperature control for safety food, disposition (p)
Observed several TCS (Time/Temperature Control for Safety) foods held for cold holding that were past the use-by date. I observed blue cheese in the reach-in-cooler with a use-by date of 3/4, chili in the reach-in-cooler with a use-by date of 5/27, cheese sauce in the steam well with a use-by date of 5/26, and a foot long hot dog in the refrigerator with a use-by date of 5/27. CA: All TCS food held for cold holding must be discarded after 7 days. COS: All food past the use-by date was discarded.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed vent hood covered in grease and grime accumulation. Clean vent hood more frequently.
