Observed bulk can opener and microwave not cleaned at a frequency to maintain food contact surfaces clean to sight and touch. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch.

Observed sliced tomatoes and cut lettuce cooling in prep cooler. CA: Items cooling shall be cooling in approved methods. COS: Noodles placed in walk-in freezer to rapidly cool.
