Observed no notification to customers on the presence of allergens in unpackaged food items served or sold. CA: The permit holder shall notify consumers by written notification of the presence major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. COS: Poster with allergens listed provided to establishment.

Observed shelved in walk-in cooler rusting with epoxy peeling off. CA: Equipment must be in good repair. Shelving needs to be replaced. PIC will repair shelving by correct by date.

Update: Site visit made 02/25/2026. Same shelving unit in walk-in cooler. PIC indicates new units have been ordered.

Update 2: Site visit made 04/20/2026 and new shelves had been installed in walk-in cooler.

Observed no drain plug in dumpster. CA: Drains in receptacles and waste handling units for refuse, recyclables and returnable shall have drain plugs in place.

Update: Site visit made 02/25/2026 and dumpster plug had been replaced.

Observed employee wearing watch on wrist, and employee wearing bracelet on wrist while preparing/handling food in main kitchen area. CA: Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: PIC instructed employees to remove jewelry.

Observed employee preparing food without proper hair restraint. CA: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: PIC provided hair restraints.

Observed chicken nuggets and spicy chicken fries holding at a temperature less than 135F. CA: PIC chose to discard food items.

Observed employee wearing watch on wrist, and employee wearing bracelet on wrist while preparing/handling food in main kitchen area. CA: PIC instructed employees to remove jewelry.

Observed cooked beef patties holding at temperature less than 135F. CA: PIC stated TCS items had been out less than 20 minutes, therefore 1 container reheated to proper temperature. PIC chose to discard other.

Observed multiple employees handling food without hair restraints in main kitchen. CA: PIC provided employees with hair nets & hats.

Observed employee handling food with watch on wrist, and employee washing dishes with bracelet on wrist. CA: PIC had employees remove jewelry.

Observed employee handle food with acrylic painted nails without gloves. CA: PIC instructed employee to wash hands and put on gloves.

Observed multiple employees handling food without a hair restraint. CA: PIC provided employees with hair restraints.
