Observed no CFSM certification posted in public view, and no record of certification in facility. CA: The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. PIC notified to have certification posted.

Observed employee wearing multiple bracelets on wrist while preparing food in main kitchen area. CA: Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: PIC had employee remove bracelets.

Observed sanitizer solution in main kitchen area with a concentration higher than manufactures recommendation. CA: Cloths in-use for wiping counters and other equipment surfaces shall be: Held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n). COS: PIC had solution remade to proper concentration.

Observed personal cell phone on table next to food items in main kitchen area. CA: Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. COS: PIC relocated item to designated area.

Observed dishes wet stacked in main kitchen above dishwashing area. CA: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food. COS: PIC rewashed the dishes and air dried.

Observed dead roach on premises in dry storage room. CA: PIC to investigate daily, for evidence of roaches and have pest control plan followed regularly.
