EHS observed stains on various linens (bed skirts, mattress protectors, and fitted sheets) throughout the facility. Also observed hair and debris from previous customers between the headboard and mattress.

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

C/A: PIC was informed that the stained linens will need to be replaced

Observed dead bug on bed skirt in room 101. Another dead bug was observed between the mattress and box spring in room 201

EHS observed stained curtains in 221 and a chipped wooded nightstand in room 204.

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

C/A: PIC was informed that curtains may need to be laundered or replaced, and the nightstand will need to be discarded or repaired

EHS observed rusted, loose toilet paper holder in room 105.

511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair.

C/A: PIC was informed that the toilet paper holder will need to be replaced

EHS observed dust build up on the vent fans of all rooms and on the head boards of rooms 110 and 204.

511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean.

C/A: PIC was informed that dusting will need to be done frequently to prevent dust build-up

EHS observed mold/algal-like substance on the dispenser of the self-service ice maker.

511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice.

C/A: PIC was informed that the ice maker will need to be cleaned regularly in order to prevent substance build-up

Observed no annual gas inspection on file. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency. Contact service provider to have all gas fired equipment inspected
