Observed a tray of mixed vegetables not hot held 135°F or above stored on storage rack at room temperature in main kitchen (see temp log).
CA: TCS foods shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above.
COS: PIC reheated mixed vegetables to 165 or above.

Observed pieces of raw salmon in reduced-oxygen packaging completely thawed and stored in walk in cooler.
CA: Reduced oxygen packaged fish shall be removed from the reduced oxygen environment prior to its thawing under refrigeration that maintains the food temperature at 41°F (5°C) or less; or prior to, or immediately upon completion of its thawing, using procedures to completely submerge in running water.
COS: PIC discarded salmon.

Observed food employee leave the kitchen with gloved hands, renter the kitchen opening the door, remove gloves, and don new gloves without washing his hands and begin preparing RTE food. CA: PIC must ensure all employees wash hands when changing tasks and when entering the kitchen before preparing food. COS: educated employee about when to wash hands and he washed his hands properly at that time.

Observed open cooler in serving area with food items at 50-50oF. Corrective Action. All items were discarded and discussed with Person-in-charge the need to temp all food items after an hour to ensure that unit is holding at correct temperatures as these open coolers are notorious for struggling to maintain min. temperatures. Person-in-charge stated that they would have unit looked at ASAP and if need to be replaced would seek a closed unit (with doors).

Drink nozzles not being cleaned at a frequency to maintain cleanliness to sight and touch. Observed black build up on drink nozzles and tea nozzles in front service area. CA: Instructed PIC to increase the cleaning frequency of the nozzles. PIC took nozzles off to have them cleaned
