Observed cooked chicken in freezer that did not have any date marking present. CFSM must ensure that opened/prepared food has a date mark present CA: Food is discarded

Observed food employee wearing jewelry other than a plain ring on their hands/arms while preparing food. CA: PIC must ensure only 1 plain band is worn on hands and wrists

The most current inspection report is not prominently displayed in public view, within 15 feet of the front or primary public door and between five feet and seven feet from the floor or in an area where it can be read at a distance of one foot away.
17A - toilet facilities: properly constructed, supplied, cleaned

Observed toilet room door propped open at time when cleaning was not being done. CA: PIC must ensure bathroom door is closed when it isn't being cleaned COS: door was closed
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed floors with build up present causing shoe to stick to floor CA: PIC must ensure floors are cleaned properly
