No lights in the walk-in freezer. Employees have been using a handheld flashlight to see in there. CA: The light instensity shall be: at least 10-foot candles (108 lux) at a distance of 30 inches (75 cm) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning. Issue has been submitted since January of this year but the light has times where it will and won't work. If this is a persisting issue where it cannot function during hours of operation then it need to be fixed. Correct within 1 month, if more time is needed contact EHS at Pulaski Health Department.

Observed handwash sink in the dishroom not getting any hot water to it even after running for a lengthy period. Handwash sinks shall be equipped to provide tempered water at a temperature of 100 degrees. CA: Have maintanence come out and fix hot water to sink, CFSM instructed to place DO NOT USE sign on sink without hot water and redirect staff to ones that do.

Observed both receptacles in unisex bathrooms without lids/broken lids. Receptacles in bathrooms that are not designated specifically male or female must have a lid. CA: replace lids on receptacles in unisex bathroom by next inspection.
