Observed employee touch paper towel dispenser handle with gloves and immediately return to food preparation without changing gloves. C/A: Change gloves when switching tasks to prevent cross contamination.

Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed food debris and buildup on floor throughout kitchen. C/A: Increase cleaning frequency.
