Observed person in charge (PIC) cannot demonstrate in a verifiable manner that food employees are/were informed of their responsibility to report symptoms and/or illnesses. All food employees that are currently on staff must proof of acknowledgement and agreed to reporting.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(q)1&3 - good repair & calibration (c)
Observed door to exterior of fryer is in disrepair.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed that non-food contact surfaces not clean. (ex. exteriors and interiors of cook equipment, cooler, freezers; door handles, gaskets on coolers and freezers and cracks and crevices throughout kitchen).
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed floors, baseboards, and walls under and behind cook equipment is in need of detailed cleaning.
