Observed a gallon of 2% milk opened and not properly date marked in the refrigerator in the back kitchen area. TCS food shall be properly date marked when cold holding for future service.

Observed large holes in the wall in the back swinging door area that leads to the back kitchen area. These holes need to be patched in order to prevent possible intrustion of insects/rodents in the kitchen area.

Observed an open gallon of 2% milk opened and not properly date marked. Also observed a plate of sausage links and scrambled eggs not properly date marked. Time/Temperature control for safety food shall be properly date marked when cold holding for future service. Once original container of milk is opened, it should be date marked for disposal with the date 7 days from opening original container OR by the manufacturer discard date.

Observed sanitizer level at "0" in wet wiping cloth bucket. Spoke with PIC and reminded her to be sure sanitizer is at proper strength in wet wiping cloth buckets.

Observed utensils, napkins, etc. stored on the floor in the storage room area in kitchen. Utensils/food items shall be stored at least 6" off the floor.
