Observed a can of green beans in the storage area that was dented/damaged around the seal. Food packages shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants.

Observed water puddled on floor around the 3 compartment sink area. Manager stated she has been in contact with ownership. Water appears to be coming from sink or sink drain area because I did not observe water dripping from pipes underneath. Plumbing in facility shall be maintained.

Observed puddled water on floor of walk-in cooler (cole slaw cooler unit). Also observed that the latch that seals door tightly shut is not working properly, which could lead to a condensation issue, thus the puddled water on the floor of the unit. Management has informed owners of need to repair the latch.

Observed lids on outdoor garbage dumpster left open and a lot of trash/debris on the ground behind the dumpster. Lids shall remain closed when not in use and trash shall be INSIDE dumpster in order to prevent intrusion or infestation of insects and rodents.

Observed large crack around piping going through the ceiling in the dry storage area that is in need of spray foam or caulking to prevent possible intrusion of insects/rodents or collections of dust/debris from attic area.

Observed two mops stored in the mop buckets, not hanging up to air dry between uses. PIC will see what she can do about getting fixed so they can hang mops.

Observed puddled water on floor of walk-in cooler beside entrance from cooler into freezer and observed ice on the floor of the walk-in freezer. PIC stated that they had some condensation recently and had it repaired. Will check into having checked and repaired again. Equipment shall be designed and constructed to be durable and retain their characteristic qualities under normal use conditions.

Observed an accumulation of food debris in the warmer drawer in the front food service area. Observed an accumulation of dust on the ice machine vent. Observed a heavy buildup on the front glass door area of the "Henny Penny" oven in the fryer area. Observed food and grease accumulations on the sides of the KFC warmer in the fryer area. These surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. PIC stated she would share with manager and get it taken care of.

Observed kitchen ceilings with accumulations of dust, walls behind fryers with accumulations of grease and residue, and soiled floors around the back handwashing sink. All facilities shall be maintained in good repair, clean to sight and touch, and easily cleanable. PIC stated she will work on these things today.
