2-2B - proper eating, tasting, drinking, or tobacco use

Employee drink stored on prep table. Employee drinking from gatorade/water glass. Advised PIC employee drinks shall be stored in a designated area away from food and food contact areas and employees shall drink from a ss cup with a secure lid and straw so that it is handled to prevent contamination of the employees, hands, the container, exposed food, clean utensils and unwrapped ss items.
12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(q) - food storage (c)
Food (potatoes) stored on floor in storage room. Food stored on floor in WIF. Advised PIC to relocate. Food shall be stored at least 6 inches off floor to prevent contamination.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Employee preparing food with no hair restraint. Advised PIC to put on hair restraint approved by the health authority. cos-put on cap
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
In use wet wiping cloths stored on prep tables. Advised PIC to relocate to sani buckets. In use wet wiping cloths shall be stored in sanitizer buckets at proper concentration between uses.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Shelves holding plates, bowls, etc. dirty. Large metal and plastic containers holding bottles, lids, utensils accumulating food and trash. Inside coolers and freezer dirty with food build up. Advised PIC to clean.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Floors throughout kitchen under equipment and shelving dirty. Advised PIC to clean.
