High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed lack of back flow device on the non chemical side of the splitter in the mop sink. Owner tried to switch hoses on the splitter, back flow was sealed in place. Corrective Action Taken Repeat Violation Admin Complaint

Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Observed lack of a plan. One was emailed.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the soda nozzles have buildup.

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed : lack of paper towels in the dispenser on the front line. Paper towels were added. Corrected On-Site

Basic - Buildup of food debris/soil residue on equipment door handles. Observed : the 2 door reach-in cooler coolers handles have flour and a moist substance in/on the handles. These were cleaned by staff. Corrected On-Site

Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed : a jacket on the front line stored on the burlap bags with baked goods.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed : the bulk containers in the hallway have an accumulation of soil/debris on the exterior. ALSO, the red lids for bulk items on the front line are soiled. And the drain for the soda machine is soiled.

Basic - Sanitizing solution for wiping cloths not free of food debris and visible soil. Observed 2 soiled sanitizer solutions on the front line.

Basic - Single-service articles improperly stored. Observed : c-fold paper towels stored on the floor by the 2 door reach-in cooler.

High Priority - Live, small flying insects found. 1 fly flying around bagel making area of kitchen.

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Missing at mop sink at the end of the splitter with black hose attached.

Manager made a sign indicating "Nova" is raw salmon, asterisk will be added to menu once shop is closed and employee can climb onto counter to update menu. Corrected On-Site
