High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the glass door reach in cooler, milk (44F - Cold Holding) per operator had out prior to inspection. Ambient air of reach in cooler at 38F.

High Priority - Toxic substance/chemical improperly stored. Fly bait stored on ice crushing machine. Operator voluntarily discarded. Corrected On-Site
