Observed multiple employees food safety training certificates expired. Operator able to provide one employee certificate that was not expired.

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed clean stacked pans on storage shelf in warewashing unable to properly air dry

Basic - Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in mop sink in a manner for mop not to properly dry. Repeat Violation

Basic - Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed Sanitizer Bucket (Quaternary 0ppm) at prep station. Operator emptied bucket and remade sanitizer solution. Retested at 200 ppm Corrected On-Site

High Priority - Employee washed hands with cold water. Employee washed hands in hand wash sink with hot water turned off then prepared pizza. Discussed utilizing opposite cook line hand wash sink until repaired. Corrective Action Taken

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Pizza oven tracks have build up of food debris.

Intermediate - Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Hot water turned off at hand wash sink by dough station due to leak when turned off. Discussed utilizing opposite cook line hand wash sink until repaired.

Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employees jacket over single service napkins. Employee speaker over clean utensils opposite three compartment sink.

Basic - Bathroom facility in disrepair. Employee restroom out of order during inspection. Observed mold like substance on wall and in cabinet under sink. Person in charge states is due to be redone. Are utilizing customer restroom.

Basic - Water leaking from pipe and/or faucet/handle. Leak observed under triple sink. Person in charge placed catch bucket. Also, cold handle missing from hand wash sink by walk in. Also, caulking around hand wash sink by walk in with mold like substance. Not smooth and easily cleanable. Repeat Violation

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Hose bibb outside back door with hose attached missing Backflow Preventer.

Intermediate - Handwash sink used for purposes other than handwashing. Cleaning supplies stored in hand wash sink by triple sink. Items were removed. Corrected On-Site
