Intermediate - Spray bottle containing toxic substance not labeled. Observed unlabeled spray bottle of yellow liquid in dish area.

Basic - Cove molding at floor/wall juncture broken/missing. Observed broken tile at floor/wall juncture next to back door in kitchen.

Basic - Dead lizard on premises. Observed 2 dead lizards on floor of walk in cooler in back of restaurant. Employee swept up and sanitized the area.

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed wall behind dish machine with mold like substance.

High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed whipped butter at 80F in walk in cooler that was turned off overnight.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed whipped butter at 80F in walk in cooler that was turned off overnight.

Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed liquid eggs in reach in cooler with no date marked held overnight.
