Basic - Employee with ineffective hair restraint while engaging in food preparation. Food work with long hair and visor and one pony tail. Hair has movement with current restraint

Basic - Floor not cleaned when the least amount of food is exposed. Multiple areas through out location with debris from old food and/or trash, under pallet,behind washing machine in pan to water heating tank. Wall seem from walk in cooler to back wall needs sealed, pipe from water line of walk in cooler with large gap around hole in wall. Wall under 3 compartment sink soiled. Repeat Violation

Basic - Ambient air thermometer not located in the warmest part of the cold holding unit. -only ambient air thermometer in walk-in cooler is in the coldest part Repeat Violation

Basic - Garbage on the ground and/or pad around dumpster.

Basic - Open dumpster lid.

Basic - Wall soiled with accumulated grease, food debris, and/or dust. -wall around handwash sink next to walk-in cooler has an accumulation of grease and food debris

Basic - Water leaking from pipe and/or faucet/handle. -triple sink faucet leaks while in operation, operator states parts have been ordered

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Splitter in with vacuum breaker in place, but on wrong threaded split.

Basic - Ambient air thermometer not located in the warmest part of the cold holding unit. Ambient air thermometer on door broken, second ambient at condenser unit at coldest part of the walk in cooler, operator added additional ambient near door. Also in the back of the make table, needs to be brought to the front

Basic - Unclean building components, attachments or fixtures. Drain line from walk in cooler soiled.

High Priority - Hot water takes too long to reach handwash sink resulting in employees using only cold water to wash their hands. Observed team member wash hands while waiting for manager, inspector used same hand wash sink within 3 mins, took over 4 mins for water to reach 100°.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust.small speed rack holding prepped pizza soiled.

Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface.
