Basic - Equipment in poor repair. Observed all gaskets in the glass door reach-in cooler worn and torn.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed in the reach-in cooler, ground beef (46°F - Cold Holding). Per the manager the item was in the cooler overnight. Operator voluntarily discarded the item.

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.

Intermediate - No probe thermometer provided to measure temperature of food products.
