High Priority - - From initial inspection : High Priority - Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. Observed salmon and hamachi offered raw for sushi missing parasite destruction letter. Advised operator to fully cook fish or discard. Observed 12 rice cake with pork Vacuum packaged on site (67F); 5 bags of pork floss (cooked pork) Vacuum Packaged (67F) per operator items are made on site and then vacuum sealed and stored at room temperature for sale. Per operator items were made on Monday and stored for customer self service. Observed fried onion (68F) on top of make table in rear kitchen area. Per operator homemade onsite and seasoned and fried in hot oil and stored above make table since Monday. See Stop Sale.
