Intermediate - Handwash sink not accessible for employee use at all times. Observed hand washing sink located in kitchen area next to 3 compartment sink inaccessible with towels and containers in sink. Observed hand washing sink inaccessible with towels and utensils at cook line.

High Priority - Roach activity present as evidenced by live roaches found. Observed 1 live roach crawling on water heater beside dry storage shelf in kitchen area. Observed roach egg on floor located by water heater in kitchen area. Observed roach egg ion floor located in dining room.
