High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no vacuum breaker on hose Bibb.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed cutting boards through out kitchen with black mold like substance build on them. Repeat Violation Admin Complaint

Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed in walk in cooler prepped marinating chicken, milk, and other items not date marked when prepped or opened. Repeat Violation Admin Complaint
