Intermediate - Required employee training expired for all employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. The operator was unable to locate the current employee training certificates.

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed the interior of the ice chute at the self-serve water station soiled with a black mold-like substance.

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. On an employee engaged in food preparation wearing a watch. The employee removed the watch. Corrected On-Site

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime.observed cutting boards on line with food stains on them.

Intermediate - Handwash sink not accessible for employee use at all times. Observed handwashing sink next to reach in and fryers blocked with broom and dustpan.

Basic - Cutting board has cut marks and is no longer cleanable. Observed on make line, cutting boards with grooves on them.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up in walk in freezer.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets on line soiled.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed around ice bin in drive thru black mold like substance

Basic - Buildup of food debris/soil residue on equipment door handles. Observed door handles to reach in coolers soiled with food debris

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up on floor of freezer all the way in the back and ice forming on ceiling of walk in freezer

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Located at inside mop sink
