Basic - Ceiling tile missing. Observed cut out in ceiling tile at electrical box, not sealed off from exposure of roof.

Basic - Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored mop head down in mop sink. Observed operator storing mop correctly with mop head up.
