Rehydrated onions at grill with no time marking. Manager states onions just brought out from walk in cooler and labeled appropriately. Corrected On-Site

Multiple ceiling tiles and vents in kitchen, ware washing and storage areas are soiled. Floor under fryer with grease and debris accumulation. Fan guards in walk in cooler are soiled.

Basic - Outer openings not protected with self-closing doors. Side customer entrance door does not self-close tightly leaving a gap to outside.
