Basic - Wall soiled with accumulated grease, food debris, and/or dust. Near walk-in cooler, cardboard covered wall area has accumulated grease

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Leak in ceiling near triple sink technician has been called per operator Corrective Action Taken

Basic - Exhaust system operated with filters removed. No means to capture grease and/or condensation to prevent dripping. Fryer hood missing filters and in operation
