Basic - Employee with no beard guard/restraint while engaging in food preparation on the dining room assembly side.

High Priority - Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Triple Sink (Quaternary 100ppm). Operator directed employee to drain and refill sanitizer compartment.
