High Priority - Operating with an expired Division of Hotels and Restaurants license. Observed the food service license had expired on 12/01/20. Admin Complaint

Intermediate - No soap provided at handwash sink. Observed no soap at the hand wash sink in the unisex employee rest room. Operator supplied the soap. Corrected On-Site

Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed a vape pen in the walk in cooler.

Basic - Employee with no hair restraint while engaging in food preparation. Observed the employees preparing food with no hair restraints.

Basic - Establishment increased seating without providing written approval from the local fire authority and wastewater provider. Observed a increase of seats from licensed one to twenty-six seats inside and nine seats outside for a total of thirty-five seats. This is a repeat violation from 09/22/20. Discussed with operator updating the food service license to the correct amount of seats. Repeat Violation

Basic - Water leaking from pipe and/or faucet/handle. Observed a leaking pipe at the three compartment sink.
