Basic - Cutting board has cut marks and is no longer cleanable. Observed cutting board at prep table is grooved.

High Priority - Server handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Observed operator handling wiping cloth then add gloves and handle ready to eat foods without washing hands. Operator was advised then washed hands before resuming.

Intermediate - Employee with long fingernails working with exposed food, single-service items or clean equipment and utensils. Observed operator with long fingers preparing foods and dipping ice and handling single service items.

Basic - Bowl or other container with no handle used to dispense food. Observed plastic cup used to scoop sugar ,no handle,
