Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Nozzle of soda machine in lobby, employee began to clean during inspection. Corrective Action Taken

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. At hand wash station near mop sink. In women's bathroom. Employees began to refill dispenser's during inspection.

Basic - Dead insects on premises. Approximately 12 dead flies and ants in unused drive through area near triple sink.
