High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee on cell phone, put gloves on without washing hands, then touched clean utensils on cook line. Operator discussed with hand washing procedures with employee. Corrective Action Taken

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. cut cabbage, cut tomatoes, house made salsa, goat cheese, cooked egg (49F-53F - Cold Holding) in top of reach-in cooler across from glass door cooler on cook line. Operator stated equipment was just worked on yesterday. Operator had employee create an ice bath and placed it under TCS foods in top of reach-in cooler to chill.
