Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. ROP salmon thawed before being reintroduced into oxygen.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Self service soda machine.

Basic - Equipment and utensils not properly air-dried - wet nesting. Containers wet nesting near dish machine.

Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Utensils in standing water temped 128 F. Manager removed utensils at time of inspection. Corrected On-Site

Basic - Old labels stuck to food containers after cleaning. Old labels on clean containers near dish machine.

Basic - Single-service articles not stored inverted or protected from contamination. Aluminum lids not inverted, employee inverted at time of inspection. Corrected On-Site

Basic - Wet mop not stored in a manner to allow the mop to dry.
