Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed stained cutting boards on the cook line.

Intermediate - Handwash sink not accessible for employee use at all times. Observed a handwashing sink that was completely turned off and taped up. Manager turned water back on and made accessible agin. Corrected On-Site

Intermediate - No soap or hand washing sign provided at handwash sink in the front service area.

Intermediate - Spray bottle containing toxic substance not labeled. Observed spray bottles with cleaning products with no labeling. Discussed proper labeling with employee.
