Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed employee wash hands in the triple sink. I told him that he had to wash his hands in the hand sink. Observed the employee wash his hands in the hand sink. Corrected On-Site

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up in the front freezer chest.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the fan over the stove with debris on the cover.

Intermediate - Handwash sink not accessible for employee use at all times. Observed a towel bar stored inside the sink. Manager removed the items from the sink. Corrected On-Site

Basic - Dead roaches on premises. Observed: 8 dead roaches at the triple sink 2 dead roaches at steam table 3 dead roaches at the refrigerator Manager cleaned up the dead roaches.

Basic - Single-service articles improperly stored. Observed to go containers stored on the floor. Manager moved the containers to the shelf. Corrected On-Site
