Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at the hand wash sink next to the three compartment sink. Manager had an employee replace the paper towels. Corrected On-Site
Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed two ceiling tiles with water stains in the hallway walking towards the bathrooms.
