Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed black substance on the interior of the ice machine bin.

Basic - Employee with no beard guard/restraint while engaging in food preparation. Observed employee with no beard guard. Operator put on beard guard. Corrected On-Site

Basic - Employee with no hair restraint while engaging in food preparation. Observed employee with no hair restraint. Operator put on hat. Corrected On-Site

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed build up of ice in the walk in freezer .

Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Observed no cover on the light in the walk in freezer.

Basic - Employee with no beard guard/restraint while engaging in food preparation. Observed employee preparing food with no beard guard protection. Repeat Violation

Basic - In-use knife/knives stored in cracks between pieces of equipment. Observed knife stored between sink and wall next to second roll top in the kitchen cook area. Operator removed knives. Corrected On-Site

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at the handwash sink next to the ice machine. Operator replaced towels. Corrected On-Site

Intermediate - Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Observed establishment is cooking eggs to order without a consumer advisory posted or on the menu. Provided operator printed copy and emailed a copy to the owner. Corrected On-Site

Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed bulk bus tubs of rice, salsa, beans, pulled chicken and pulled pork prepared over 24 hours ago according to the operator not date marked.

Basic - Employee with no beard guard/restraint while engaging in food preparation. Observed employee preparing food with out wearing a beard guard.

Basic - Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed operator was washing and rinsing dishes without sanitizing. Spoke with operator regarding proper sanitation methods and operator filled sanitizer sink up to properly sanitize. Corrected On-Site

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed scoop handle in contact with sugar in, in use bulk sugar bin.

Basic - Equipment in poor repair. Observed gaskets on reach in cooler unit next to hand wash sink with torn and missing gaskets.

Basic - Floor soiled/has accumulation of debris. Observed floor soiled under bulk storage shelves next to walk in cooler.

Basic - Food not stored at least 6 inches off of the floor. Observed onions and tomatillos stored on shelf in dry storage area which is not at least 6" off of the floor.

Basic - In-use wet wiping cloth/towel used under cutting board. Observed wet wiping cloth under cutting board next to reach in cooler on prep side of kitchen across from the grill. Operator removed towel. Corrected On-Site
