Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at the handwashing sink next to the 3 compartment sink. Operator replaced towels. Corrected On-Site

High Priority - Spray hose at dish sink lower than flood rim of sink. Observed spray hose at prep sink hanging below steel dividers. Operator states they have a work order out to have it repaired. Corrective Action Taken

Basic - Employee with no beard guard/restraint while engaging in food preparation. Observed employee preparing food without beard guard.

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed scoop handle in contact with chicken salad in reach in cooler. Operator removed scoop. Corrected On-Site

High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee put on gloves without first washing hands.
