Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed chicken and eggplant Parmesan prepared over 24 hours according to operator not properly date marked when placed into the freezer on the Pizza side in the reach in freezer.

Basic - Bowl or other container with no handle used to dispense food. Observed plate being used in panko breadcrumb bag as a scoop.

Basic - Buildup of food debris/soil residue on equipment door handles. Observed build up of food debris on reach in door handles near dough mixer.

Basic - Carbon dioxide/helium tanks not adequately secured. Observed carbon dioxide tanks near rear door not secured.

Basic - Employee eating in a food preparation or other restricted area. Observed glass of water on Pizza prep table next to reach in cooler in front of pizza oven.

Basic - Employee with no hair restraint while engaging in food preparation. Observed employee on Pizza side not wearing a hair restraint.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed buildup of food debris on reach in cooler gaskets near dough mixer.

Basic - Wet mop not stored in a manner to allow the mop to dry. Observed wet mop on floor next to mop sink.
