Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Water bottle on shelf above a prep table. Operator removed bottle. Corrected On-Site
Basic - Employee with no hair restraint while engaging in food preparation. Observed a line cook and a dish washer without hair restraints.

High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Observed cooked prime rib in the walk in cooler at 50° F Per the operator prime rib was cooked the previous day
High Priority - Dented/rusted cans present. See stop sale. Observed three 20oz cans of Dole Pineapple Rings with rust on lids and rims.
High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed a reading of 0 ppm on dish machine. Operator filled sanitizer in three compartment sink. Observed a reading of 100 ppm in sanitizer compartment. Ecolab technician arrived during the course of inspection and repaired the dish machine; inspector verified the machine was sanitizing with chlorine at 50 ppm. Corrected On-Site
High Priority - Stop Sale issued on time/temperature control for safety food due to temperature Observed cooked prime rib in the walk in cooler at 50° F; per operator prime rib was cooked the previous day.

Observed slicer machine in the dry storage area with what appeared to be food debris between the blade guard and the blade. Operator covered the slicer and marked it "out of use" until unit can be cleaned Corrective Action Taken
Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed the ice shield inside the ice machine to have a mold like substance. Operator cleaned the ice machine while inspector was on site. Corrected On-Site
Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup inside the walk-in freezer.
Basic - Wet mop not stored in a manner to allow the mop to dry. Observed mop not inverted in order to dry properly.
