Basic - Exterior door has a gap at the threshold that opens to the outside. Observed gap under rear door to outside next to mop sink room.

Basic - Floor tiles missing and/or in disrepair. Observed missing tiles inside walk in freezer at the threshold. Operator stated maintenance is aware and will be fixing soon.

High Priority - Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee put on eyeglasses wearing single use gloves and proceed to make an egg sandwich while wearing same gloves. Discussed proper glove usage with operator. Corrective Action Taken
