Basic - Covered waste receptacle not provided in women's bathroom. Observed uncovered receptacles in woman's bathroom stalls.

Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed open mugs of coffee on prep table next to slicer. Operator removed mugs. Corrected On-Site

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed cook on cook's line wearing bracelet while preparing food. Repeat Violation

Basic - Water draining onto floor surface. Observed water draining onto floor from drain under ice machine. Operator has plumber on site working on floor drains. Corrective Action Taken

High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee pick up raw chicken with gloved hands and place on grill then plate ready to eat chicken quesadilla using same gloves holding quesadilla to cut. Discussed proper glove usage and hand washing with operator.

High Priority - Toxic substance/chemical improperly stored. Observed spray bottle of windex stored next to clean plate ware next to reach in freezer at end of cook's line. Operator removed spray bottle. Corrected On-Site

Basic - Carbon dioxide/helium tanks not adequately secured. Observed CO2 tank unsecured at outside bar.
