Observed employee touch hair then place single serve wax paper into plastic holding containers for food items without washing hands first. Discussed hand washing practice with operator. Basic - Drain cover(s) missing. Observed drain cover removed from mop sink floor drain.

Basic - In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed handle for scoop touching scrambled egg mixture in hot holding case between ice machine and fryers. Operator had employee remove scoop from food. Corrected On-Site
