High Priority - Stored food not properly protected from contamination. Observed dead flying insect inside container of milk powder. Container lid damaged and unable to fully enclose food product. Employee threw out food and stated manager had ordered new tops for containers. Corrective Action Taken

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed two cutting boards with mold like substance. Employee threw cutting boards away. Corrective Action Taken

Intermediate - Handwash sink used for purposes other than handwashing. Observed employee wash coffee filter holder out in hand wash sink in server area.

Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed grouper thawed in ROP packaging with no air escaping. Repeat Violation

Basic - Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Observed fly trap stored above reach in cooler with approximately 50 flies. Operator cleaned and moved fly trap to appropriate location during inspection. Corrected On-Site

Basic - Bowl or other container with no handle used to dispense food. Observed container with no handle used for scooping inside bucket of milk powder.

Basic - Clean utensils or equipment stored in dirty drawer or rack. Observed clean utensils stored in two soiled bins at dish area. Operator stored clean utensils in clean bins during inspection. Corrected On-Site
