Operator states it had been out for approximately 30 minutes and tossed it of their own volition. Corrective Action Taken

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed paint peeling from drop ceiling supports above prep line in kitchen. As a repeat violation the importance of following proper protocol with this violation was reinforced with the operator. Repeat Violation

Basic - Ice buildup in walk-in freezer on the floor next to door.

High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Employee beverage container in ice bin on northern side next to drive thru window. Operator discarded all the ice which was approximately 5 lbs.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed drop ceiling metal fixtures peeling paint on kitchen ceiling.

Basic - Drain cover(s) missing. Observed drain cover missing beneath three compartment sink.

Basic - Employee beverage container in ice bin on northern side next to drive thru window. Operator discarded all the ice which was approximately 5 lbs.

Basic - Ice buildup in walk-in freezer on the doorframe and under the condenser.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed inside door frames of walk in cooler and walk in freezer soiled with dirt.

Basic - Observed floor tile missing under left fryer in kitchen.

Basic - Open dumpster lid. Operator closed the lid. As a repeat violation the importance of following proper protocol with this violation was reinforced with the operator. Corrected On-Site Repeat Violation

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed wall soiled on top lip of cove molding around the kitchen.
