Basic - Bowl or other container with no handle used to dispense food. Observed cups used as scoops in their dry storage goods. Operator removed them. Corrected On-Site

Basic - Missing drain plug at dumpster. Observed their drain plug ripped off.

Basic - Outer openings not protected with self-closing doors. Observed their back exit door not self closing.

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no vacuum breakers on Y fitting at mop sink.

Basic - Clean utensils or equipment stored in dirty drawer or rack. Observed knives stored between shelves on rack above three part sink. Manager removed knives. Corrected On-Site

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at front hand sink. Manager replaced.

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed stacked wet pans on shelf across from three part sink.

Basic - Exterior door has a gap at the threshold that opens to the outside. Observed gap bottom left corner of rear exterior kitchen door.

Basic - Working containers of food removed from original container not identified by common name. Observed squeeze bottles not labeled at front food line.

Basic - - From initial inspection : Basic - Equipment in poor repair. Observed pan set up in Walkin Cooler to catch water leaking from condenser. Warning - From follow-up inspection 2022-09-22: Observed pan set up to catch water from leaking condenser. Operator still trying to find someone to repair. Time Extended

Basic - Ceiling tile shows damage or is in disrepair. Observed ceiling tile above walk in freezer peeling back from the corner.

Basic - Floor soiled/has accumulation of debris. Observed food debris on floor under front counter in the corner. Operator cleaned floor. As a repeat violation the importance of following proper protocol with this violation was reinforced with the operator. Corrected On-Site Repeat Violation

Basic - Ice buildup in walk-in freezer. Operator has a work order to replace the gaskets. Corrective Action Taken
